Kindergarten Registration and Assessment
Parents of children eligible for Kindergarten should call the Board of Education Office at 937-964-1318 to arrange an appointment.
Documents Needed to Register:
Child’s Birth Certificate (Health Department Copy)
Child’s Immunization Record
Child’s Custody/Adoption/Guardianship Papers (if applicable)
Proof of Residency (Parents)
Parents ID
Please, be reminded Kindergarten Students must be 5 years of age by August 1st. State Law requires that all students must attend Kindergarten before entering First Grade.
